Client Recording Consent Form

Coaches often share a session recording with their mentor coach so the mentor coach can review it and help the coach develop their skills. This form records the coaching client’s permission before any recording is shared. Read each statement, ask your coach about anything that is unclear, and sign only if you agree.

What I agree to

Check each statement you agree to. All five are needed for the recording to be shared.

Sharing. My coach may share the recording with their mentor coach, who will review it to help my coach develop their skills. Secure storage. The recording may be uploaded to secure storage that my coach and their mentor coach can access. It will be kept for the period written below and then deleted. Transcription. The recording may be converted to audio and transcribed into written text for the review. AI-assisted processing. AI-assisted tools may process the transcript to help organize possible observations. The mentor coach, not the software, remains responsible for every professional judgment. Questions and withdrawal. I may ask my coach questions before agreeing. I may later withdraw this consent or ask about deletion. Withdrawal does not undo review already completed or remove records that must lawfully be kept.
